Start from a Sample

Updated Oct 8, 2026· Custom Forms

You don't always have to build a Custom Form from a blank canvas. iReport includes a few ready-made sample forms for common safety documents — start from one and you get a complete, working layout you can adjust to fit your team, instead of building every field yourself. Starting a form is an admin task, done from the Custom Form Templates area.

Available samples

  • Job Safety Analysis (JSA) — a standard JSA: general info, a job-steps / hazards / controls table, required PPE, hazard assessment, and a supervisor sign-off.

  • Job Hazard Analysis (JHA) — a task-by-task hazard breakdown with a per-step risk ranking, required PPE, permits & authorizations, and a supervisor approval / sign-off.

  • Sign-In Sheet — an attendee roster for a meeting, toolbox talk, or site visit — name, affiliation, and time in / out, with a facilitator certification signature.

Start a form from a sample

  1. In the left navigation, open Custom Form Templates.

  2. Click New Form in the top-right corner.

  3. In the New Form window you'll see a Blank form tile alongside a tile for each sample. Click the sample you want to start from.

The sample opens in the builder right away as a brand-new Draft — a full copy you own. The original sample is untouched, and nothing is visible to your team until you publish.

Make it yours

Because it's a full copy, everything is editable — rename the form, add or remove fields, adjust the layout, and change the page setup exactly as you would with a form you built from scratch. See Building a Custom Form for how the builder works. When it's ready, click Publish to make it available to your team.

Good to know

  • Starting from a sample creates an independent copy — editing your form never changes the original sample, and you can start as many forms from the same sample as you like.

  • A sample-based form still starts as a Draft. It isn't available to fill out until you publish it.

  • Prefer to start clean? Choose Blank form in the same window instead.
