Creating/Managing Lockout/Tagout (LOTO) Templates

NOTE: Only administrator users have the ability to add/edit LOTO templates.

NOTE: Safety Audit, Quality Audit, and Lock Out Tag Out Procedures Templates are located in their own separate sections.

Creating a New LOTO Template

2. Select the company level and navigate down to Audits and select Lock Out Tag Out Templates.

3. You will be taken to the Templates list page. Here you will:

  • See the list of all previously-created audit templates

  • Create audit templates

  • Duplicate existing audit templates

4. To create a new LOTO template, click in the top right corner where it says Page Actions. You will then select Add LOTO Procedure Template.

5. You will then be taken to the Create New Template page. Each template will include:

  • Title: A required field, be clear and concise as this title is reflected in Analytics

  • Description: This is only visible to admin users, to help you differentiate between templates.

  • Instructions: This will display to the employee performing the audit.

  • Has an Involved Employee(s)?: Checking this box allows you to tie the assignment of the audit to specific employee(s).

  • Is this a Confidential (_______) LOTO Procedure?: Checking this box allows you to limit who has access to this audit once submitted.

  • Allow Bulk Actions: Checking this box will allow users to mark whole sections as โˆš, X, or N/A

  • Require Photos for Unacceptable Items: Checking this box will require users to supply a photo of the question marked with X.

  • Require Notes for Unacceptable Items: Checking this box will require users to supply notes on the question marked with X

  • Require Corrective Action for Unacceptable Items: Checking this box will require users to assign a corrective action for the question marked with X.

  • Add Section: Located in the bottom left corner of the screen. One section is required for every audit, but you can add as many sections as needed.

  • Move (section): You can arrange each section as needed using the up and down arrows.

  • Options (section): This is for cloning or deleting a section.

  • Section Type: Can be either Informational or Scorable.

    • Informational sections cover a wide variety of items you may need to record in the Audit.

    • Scorable sections cover the items that you are checking to ensure Safety or Quality.

      • NOTE: Every audit (Safety, Quality, or LOTO) requires at least one scorable section with one scorable question (Can only use the Yes/No (Checkmark/X) question on a scorable question).

    • You have the ability to use the following question answer types: Yes/No (Checkmark/X), Dropdown, Multiselect, Text Field, Employee List, Table, Signature, Date, Date/Time, and Block of Text.

    • All Sections can be marked as REQUIRED, or you can mark specific questions as REQUIRED.

6. Build out the template per your company's requirements. Templates require the following:

  • At least one Section

  • At least one Question per section

    • NOTE: Every audit (LOTO) requires at least one scorable section with one scorable question (Can only use the Yes/No (Checkmark/X) question on a scorable question).

  • If you need to add more questions, click on either Add Row or Add 5 Rows.

  • Repeat Step 6 until all sections and questions have been entered and you are ready to save.

Below is an example of a completed LOTO template before saving:

7. If you are satisfied with the template you have created, then hit the green Create button at the bottom of the page!

8. You will then see your audit on the Safety Audit Templates list page, and it is available to be assigned to employee(s) or to a QR code for on-demand completion.

Managing Existing LOTO Templates

1. If you ever need to edit questions, adjust the required actions, duplicate a template, or delete a template, you will go once again to the Lockout Tagout Templates list page as described in steps 1-3 in the above section

2. The editing and deletion of LOTO templates is done within the template itself. Find the template you wish to work on and click into it.

3. Once in the template you wish to work on, click Page Actions -> Edit Template:

4. With the template unlocked, you can edit a number of things:

  • Title, description and instructions.

  • Types of actions (involved employee, confidentiality, bulk actions, photos, notes, and corrective actions)

  • Add sections

  • Mark sections or questions required

  • Add/remove questions (Add Row, Add 5 Rows, Clone/Delete)

  • Edit section titles/Delete sections

  • Edit question text

5 . If you need to delete a section, simply click Options -> Delete Section.

6. If you need to duplicate a template, select Duplicate Lockout Tagout Template under Page Actions. You will then be able to edit the template as described above.

NOTE: The duplicated template title will be shown as "_________ - Copy". Be sure to name the duplicated template in a way that best reflects your organizational goals.

7. You can Archive a template as well. From the main LOTO page check the box next to the template, then under List Options select Archive Templates.

8. If you need to delete an entire LOTO template, go into the template and click Page Actions -> Edit Template to open Edit mode. Once in Edit mode, click Page actions again and choose Delete Template:

9. Once confirmed, the template will be deleted and you will be taken back to the LOTO Templates list page.
