Assigning Lockout/Tagout (LOTO) Procedures

LOTO can be either assigned to a QR code, or assigned to employees. By using the QR code, you don’t have to assign to anyone -- it gets auto-assigned to the person who scans the QR code within their iReportSource app.

Creating a QR Code for a LOTO Procedure

  1. Log in on the web at https://www.ireportapp.com/login

  2. Click into your Dashboard

  3. From your Home Page, select Audit under the Create a New section

  • A pop up will appear. Choose Lockout/Tagout Procedure.

4. Choose Scan on Demand for the schedule, and then choose the template for the LOTO Procedure. Click Create when done.

5. You are taken back to the LOTO page. From here, click on the On Demand tab to view any QR codes that have been created.

  • From here, you can click on Download QR Code to download a PDF with the QR code to print.

6. Now you can print/save your QR code and use it however you’d like! Print it on a sticker and place it on or near a piece of equipment, keep it on a laminated sheet with the supervisors, etc. Any time an employee scans the code, they are auto-assigned the audit for completion.

Assigning a LOTO Procedure on the Web

Audits are checklists that you can assign to any active user for completion. This document walks through how to use the web platform to assign an Audit.

Assigning the Audit

  1. Sign in to your account at https://www.ireportapp.com/login

  2. Click into your dashboard

  3. From your Home Page, select Audit under the Create a New section

  • A pop up will appear. Choose Lockout/Tagout Procedure.

4. You will be taken to the assignment template. Fill it out based on your desired schedule (immediate or repeating) (See above for Scan on Demand). Note all required fields indicated by a RED *.

NOTE: If your chosen Group/Establishment has Areas, you will be required to select one as well.

NOTE: Templates are provided by your organization. If you do not see what you need, please contact your Safety Manager and reach out to iReport Customer Support.

NOTE: The equipment dropdown only applies if you are using the Equipment feature.

  • Immediate Scheduling (for non-repeating, one-off inspections)

  • Repeating Scheduling (for those inspections that you need to get done on a routine basis)

NOTE: If you are assigning to multiple people, regardless of Immediate or Repeating scheduling, be sure to check the box for "Assign Individually" if needed. If left unchecked, the assignment will be treated as a group assignment, and the first to submit the audit will submit it for all assignees. If you need to assign to multiple individuals, be sure to check this box!

You have successfully created and assigned your LOTO Procedure!

If you assigned it as immediate, the assignee(s) will receive their notification of the task being assigned to them immediately. If assigned as a repeating assignmentt, they will receive a notification the next time it is due to be assigned (in the example above, every Monday).

Employees will be notified according to their notification settings (via email if we have an email for them, or via mobile pop-up notification if they have them enabled) each time they are assigned an Audit, and this will appear on their My ToDos List.

Reassigning a scheduled, repeating LOTO Procedure

If you need to reassign the scheduled, repeating LOTO Procedure, for whatever reason, please follow these steps.

1. Login like normal and go to home page of the Group/Establishment or Company Level where the audit you will be reassigning is located.

2. Go to Audits -> Lockout/Tagout Procedures.

3. Under the Repeating Tab, choose the repeating LOTO assignment you wish to reassign.

4. Choose the new assignee and hit Create once done. This assigned, repeating audit will now assign to the new user on it's next scheduled assignment date.

  • NOTE: If you cannot find your assignee, then you must check that they are activated in iReport (can log in) and have access to the Group/Establishment the audit is assigned/created through.

NOTE: If you are assigning to a group, be sure to check the box for "Assign Individually" if needed. If left unchecked, the assignment will be treated as a group assignment, and the first to submit the audit will submit it for all assignees. If you need to assign to multiple individuals, be sure to check this box!

Reassigning an active Audit

If you need to reassign the audit for whatever reason, please follow these steps.

  • NOTE: Only Collaborator and Admin level users can reassign Safety/Quality Audits.

  • NOTE: If you cannot find your assignee, then you must check that they have access to the Group/Establishment the audit is assigned/created through.

1. Login like normal and go to home page of the Group/Establishment or Company Level where the audit you will be reassigning is located.

2. Go to Audits -> Lockout/Tagout Procedures.

3. Find the procedure you are needing to reassign using the available filters.

4. Using the check box on the left side, select the procedure you wish to reassign.

5. Once selected, click where it says List Options and choose Reassign Procedure. A modal will pop up, allowing you to select the new assignee.

  • NOTE: This will change the assignee for all selected procedures to the individual chosen. Please be aware that if you have multiple assignees to change you will need to do this one audit at a time.

6. Confirm your selection and the audit will be reassigned.
