NOTE: Only collaborator and administrator user types have access to the OSHA Forms.
Log in on the web at https://www.ireportapp.com/login
Click on the location for which you’d like to view the OSHA Forms
Company Level (this is for Admin users only)
Group/Establishment
Project Level
Go to the OSHA Forms section under the General menu:
4. You’ll be taken to the OSHA 300 Log by default. Use the tabs across the top to view the log you want to view: OSHA 300, OSHA 300A, or OSHA 301
NOTE: You can choose the year you’d like to review by using the drop-down menu toward the top right corner:
OSHA 300: Injury & Illness Log
On the OSHA 300 log, you’ll see the information as it has been populated on the OSHA Compliance report component for recordables on a First Report of Injury. Once the OSHA Compliance section is started and saved, it will populate the log entry on the 300. If any required information is missing, there will be a red exclamation point to the left of the log entry. Click this to be taken to the report where you can check/add more information.
OSHA 300A: Summary
This will show the establishment/company information such as total number of cases, days away, days on restriction, etc.
You can click on the box on the bottom left to enter worker information. The hours worked can be added by an administrator user under the company settings page -- see this document for more information.
OSHA 301: Incident Report
This section contains the detailed individual incident reports. You can click on the incident you’d like to view. As with the OSHA 300, this page will also show the red exclamation point when there is information missing for the form to be considered complete:
Downloading OSHA Forms
For any OSHA Form, you can click the Download Files button to save them as an excel .csv file. This will include the Injury Tracking Application .csv, formatted specifically for upload to the osha.gov site at the end of the year.
NOTE: You will not be able to download the ITA if any OSHA-required information is missing from any of the recordable incidents.
Updating Headcount or Hours Using an Import Template
While on the OSHA Forms page, click on Page Actions in the top right corner, then select either Update OSHA Headcount or Update OSHA Hours Worked:
NOTE: Both of these function in the same way, so I will be using Hours Worked as an example but know that you can follow these same steps to update Headcount:
2. Select whether you want to update the Hours Worked by the Group/Establishment or the Area. (In many cases, you'll choose Group/Establishment). Click Download Template:
3. The Template will open in Excel, and will have any hours you have already tracked for the year populated. Make changes to any of the cells for the month/establishment you want to update, and save the Excel file as a CSV.
4. When you are ready, go back to the OSHA Forms > Page Actions > Update Hours Worked and instead of downloading the template, click the box or drag the Excel file to where it says Click or drag a file here to upload. When your file is added, click Upload:
5. You will receive a message to let you know the Hours were successfully updated:
