My Documents: Viewing & Creating on the Mobile App

  1. Log into the iReportSource app

  2. Click on the menu icon:

3. Choose Documents:

4. Click on the My Documents section:

5. Here you will see all of your previously updated documents, forms, and any safety trainings you have completed or attended that have been tracked in the system.

6. To add a document, click the "Create a New" button at the bottom of the screen and then select 'Document' from the following screen.

7. Then select 'My Documents' from the following screen.

8. Here you are given the choice to fill out a premade form, such as a Transitional Return to Work Program or a Recurrence, Relapse During Recovery & Continuing Treatment form or take/upload pictures or upload files on your device (image, document, etc.):

9. If you chose a Create Form, you’ll choose which one to create next:

10. From here, you fill out the required fields and click the Sign & Submit button:

11. If you chose Upload Personal Document, you’ll choose whether to Upload or Take a Picture. Fill in the required information and then click Submit when it becomes active.
