Safety Walks Workflow

Completing a Safety Walk on the Web

  1. Sign into your account at https://www.ireportapp.com/login

  2. Choose the location for which you’d like to complete the Safety Walk

  3. Click on the Observation button

4. Choose Safety Walk and hit Create:

5. Select an Area (if applicable)

6. If you identify a hazard on your Safety Walk, click the Add Hazard button to the left. Alternatively, if you identify an area worthy of praise, click the Add Bravo button to the left:

7. If a Hazard is found, fill out the appropriate information.

  • Click the Add Pictures button to upload photos from your computer

  • If the Hazard was immediately fixed, check the checkbox to indicate it

  • Enter the Hazard Classification

    • If OTHER was selected, briefly describe/name it

  • Enter a description

  • Enter an exact location (OPTIONAL)

  • Assign a Corrective Action (if needed)

8. If assigning a Corrective Action, fill out the form, then click Create.

  • NOTE: Only employees who are active (able to log in) in the system can be assigned Corrective Actions.

9. Repeat the actions for adding hazards/assigning corrective action as needed.

10. If a Bravo is found, fill out the appropriate information.

  • Click the Add Pictures button to upload photos from your computer

  • Enter the Bravo Classification

    • If OTHER was selected, briefly describe/name it

  • Enter a description

  • Assign the Bravo (OPTIONAL)

11. When you have completed your Safety Walk, sign your name in the box provided and hit Save Signature. You can then hit the green Create button at the bottom of the page to submit the safety walk:

The Safety Walk is now complete, and supervisors/collaborators/administrators who have access to the location for which it was submitted will receive notifications, according to their notification settings.

Completing a Safety Walk on the Mobile App

  1. Log into your iReportSource mobile app

  2. Click "Create New":

3. Then click on Observation -> Safety Walk.

4. Fill out the required information as indicated by the red * to begin the Safety Walk. Once you do so, you will be able to "Add Hazard" or "Add Bravo":

5. When adding a hazard, you can enter the following information:

  • Click the Add Images button to take or upload photos of the hazard

    • NOTE: If you take the photos within the iReportSource app, you can click on the pencil icon at the bottom left corner of the screen to annotate/draw on the picture before uploading

  • Check the box to indicate if the hazard was immediately fixed

  • Hazard Classification

  • Type in a detailed description of what you saw

  • Type in the solution for this hazard (OPTIONAL)

  • Type in the exact location for this hazard (OPTIONAL)

  • Click the Assign Corrective Action button to assign a task to an employee regarding this hazard

  • Click the Save Changes button when finished entering the hazard information

6. Repeat Step 5 for any additional hazards you find.

7. You also have the ability to add Bravos to your safety walk as a way of acknowledging a job well done by the team.

8. When adding a bravo, you can enter the following information:

  • Click the Add Images button to take or upload photos of the bravo moment

    • NOTE: If you take the photos within the iReportSource app, you can click on the pencil icon at the bottom left corner of the screen to annotate/draw on the picture before uploading

  • Bravo classification

  • Enter a description of the bravo

  • Assign the bravo

    • This is perfect if you wish to give credit for a job well done to an individual employee or group of employees.

  • Click the Save Changes button when finished entering the bravo information

9. When all hazards and bravos have been entered and you are ready to complete the Safety Walk, click on Sign & Submit:

10. Sign your name, save, and then click the Sign & Submit button to complete this Safety Walk.

You are now finished with this Safety Walk! You will be taken back to the Safety Walks main page where you can see all Safety Walks that have been completed for this location.

Reviewing Submitted Safety Walks

NOTE: Only supervisor, collaborator, or admin users can view all submitted Safety Walks in the groups/establishments they have permissions to view. Reporter users can view Safety Walks they submitted only, until they are closed.

  1. Click on the location for which you’d like to view the completed audits (admins can choose to view the company level data by clicking on the top company level)

  2. Go to the Observations menu and click Safety Walks:

4. You’ll be taken to a page that displays all Safety Walks. You can filter the list using the following options:

  • Date Range: Filters the list to show only those Safety Walks that were submitted in the chosen range.

  • Stage: Filters by the stage the Safety Walk is currently in. They are:

    • Severity Review: These walks were submitted with at least one hazard that was not immediately fixed.

    • Corrective Action: These walks have at least one hazard that has been assigned a corrective action that is not yet complete.

    • Past Due: These walks have at least one corrective action assigned with a due date in the past.

    • Closed: Walks that meet one of these criteria:

      • They were submitted with no hazards (Bravos only, or a walk with no observations made)

      • They were submitted with hazard(s) but all of them were immediately fixed.

      • They were manually closed

      • They had corrective actions assigned for all hazards, and all corrective actions are marked complete (the system closes these automatically)

  • SW Title: Filters by the title of the Safety Walk, indicated by the Group name and number.

  • Reporter: Who submitted the Safety Walk

  • Created Date: When the Safety Walk was submitted.

  • Hazard Count: Total number of hazards found

  • Bravos Count: Total number of bravos found

  • Corrective Actions Created: Total number of Corrective Actions created.

5. The Safety Walk Info & Summary will show you the inspection location, # of hazards found, # of bravos found, and the number and status of any corrective actions:

6. If you scroll down from the summary, you’ll see each individual hazard/bravo and their details:

7. Click the Corrective Action button to view an assigned corrective action, or assign a corrective action (if not already assigned):

8. If you’d like to download the Safety Walk as a PDF, click the Download Safety Walk button underneath the Summary:

9. Safety Walks will automatically update their stage from Severity Review to Corrective Action to Past Due or to Closed.

10. Admins only: You can click Page Actions -> Delete Safety Walk if you would like to delete the Safety Walk you are viewing.

  • NOTE: If you delete the Safety Walk, it will no longer be on the platform for anyone to see. If you delete one by mistake, please contact Customer Success for assistance.

Downloading/Printing Single Safety Walks

1. Log in to iReportSource on the web at https://ireportapp.com/login and choose your location

2. Click on Observations, then Safety Walks

3. Click on the Safety Walk from the list you wish to print, filtering as need to find the one you are looking for:

4. Click Download Safety Walk in the bottom right corner of the Summary Section

5. A new tab will open in your browser with a PDF of the Safety Walk you chose. In the top right you will have the options of downloading the PDF (great for email) or printing the PDF.

Downloading Bulk Safety Walks

1. Log in to iReportSource on the web at https://ireportapp.com/login and enter your dashboard.

2. On the left-side menu, click on Observations, then select Safety Walks:

3. Click Page Actions and choose Export Safety Walks:

4. Select any filters to export only the Safety Walks you want to see. The only required filter is the Date Range. Click Generate when finished:

5. Return to Page Actions. During the creation of the report, it will show Generating Bulk Report. If you are generating a large report with Safety Walks that have multiple hazards or pictures on each, it may take some time for this to generate.

6. Once it is ready, the text will change to Download Bulk Report. Click here and it will open the PDF in a new browser tab:

7. In the top right corner you will have the options of downloading the PDF (great for email) or printing the PDF.

8. Download or Print the PDF as desired.
