Completing Audits

Both Safety Audits and Quality Audits take you through a series of statements/questions you will answer in order to document items that need attention. You are also able to create corrective actions, add photos, and add notes detailing what is found.

This document takes you through the steps of starting/completing these audits on demand through the mobile app without the audit needing to be assigned to you.

NOTE: Safety Audits and Quality Audits function the same way when starting via QR Code. The example below will feature Safety Audits.

Using a QR Code to Complete an Audit

  1. Log into your iReportSource mobile app

  2. Click QR Scanner:

3. Scan the QR code:

4. You will be taken to the Audit assigned to that QR code.

  • Select the location you are performing the audit under (Group/Establishment and Area if applicable)

    • This example is from the Admin perspective being logged in at the Company Level. Most users will have their Group/Establishment answered based on which Group/Establishment they started the audit from, and will only need to identify the Area (if applicable).

  • You can add photos, notes, and assign corrective action to any question.

  • You can use the Section menu at the bottom of the page to jump to different sections of the audit, if it has multiple, or use the Back/Next buttons to quickly move between them:

5. After you have completed answering the questions, you will see the summary page. On this screen, tap Submit.

  • If you are not ready to submit, you may simply leave the audit. So long as you have been answering questions, it will auto save your progress. You can pick the audit up again later from you My ToDos.

  • Confirm your submission:

  • Email a PDF of the completed audit if needed. You can send internally (the drop down option) or externally (by adding the email manually). This step is optional, and you may simply tap Submit if no email is needed.

You have successfully completed your Audit using the QR Scanner!

Starting an Audit on Demand (Mobile)

This document takes you through the steps of starting/completing these audits on demand through the mobile app without the audit needing to be assigned to you.

NOTE: Safety Audits and Quality Audits function the same way when starting via QR Code. The example below will feature Safety Audits.

  1. Log into the iReportSource mobile app

2. Tap "Create New"

3. Choose Observation, followed by either Safety Audit or Quality Audit:

4. Choose your Audit template, Group/Establishment, and Area (if applicable). Tap Submit:

  • This example is from the Admin perspective being logged in at the Company Level. Most users will have their Group/Establishment answered based on which Group/Establishment they started the audit from, and will only need to choose the audit template and identify the Area (if applicable).

5. You will be taken to the Audit

  • You can add photos, notes, and assign corrective action to any question.

  • You can use the Section menu at the bottom of the page to jump to different sections of the audit, if it has multiple, or use the Back/Next buttons to quickly move between them:

Completing Assigned Audits on the Web

Both Safety Audits and Quality Audits take you through a series of statements/questions where you will answer a series of questions in order to document items that need attention. You are also able to create corrective actions, add photos, and add notes detailing what is found.

This document takes you through the steps of completing an Audit that has already been assigned to you on the web platform.

NOTE: Safety Audits and Quality Audits function the same way when accessing and completing an assigned audit. The example below will feature Safety Audits.

  1. Log into iReportSource at https://www.ireportapp.com/login

  2. Click on your location on the first page:

3. You will see your assigned Audits as tasks on your My To-Dos dashboard. Click on the Audit you’d like to complete:

4. There is the possibility that each individual question has a different answer type.

  • These answer types range from the Acceptable/Unacceptable/Not Applicable questions, to dropdown, multi-select, text field, employee list, date/time, table, and a signature box.

  • Be aware of this as you are answering the questions! In the example below, you will see a Dropdown and an Acceptable/Unacceptable/Not Applicable answer type.

5. Some audits may also have at the top of the section a Mark all in section drop-down, which allows you to quickly mark all √, X, N/A questions in that section with one of the √, X, N/A options.

  • NOTE: This will override any √, X, N/A answers already provided in the section. You can go back and make individual changes as necessary:

  • NOTE: Based on how the template for the Audit was set up, your organization may require the addition of photos, notes, and/or corrective actions for any item marked as unacceptable.

6. You also have the option to add photos and notes.

  • Photos lets you choose one or more photos from your device.

  • Notes allows you to record further details on what has been found.

7. If any Unsafe item needs corrective action, it can be assigned a Corrective Action during or after the submission of the Safety Audit. If you’re ready to assign the corrective action, just click the button under the appropriate question:

8. If assigning a corrective action, fill out the form that is presented, noting any required (red *) fields.

9. After corrective action has been assigned, it will show under the appropriate question on the Audit:

10. When you have completed the audit, and have added any additional notes, hit Save at the bottom.

  • NOTE: Audits are locked after submission, so please ensure you have answered all questions completely and thoroughly before saving.

  • NOTE: If you still need to work on the audit, when you click Save choose Save for Later. You can then come back to it through your My ToDos.

11. Once an Audit has been submitted, Supervisors and above can see the completed audit.

  • This shows the summary, photos/notes, answers, and scores for the section(s).

  • You can always look up an audit by following the instructions outlined under the article Reviewing Audits

1. Log into the iReportSource mobile app

2. Click on the assignment from your My To-Dos list:

3. Click on the audit you’d like to complete:

4. Using the Back/Next buttons, or the Section drop down, move through the audit answering the questions in each section as you go.

5. Some audits will have at the top of the section a Mark all in section drop-down.

  • This allow you to quickly mark all √, X, N/A questions in that section with one of those options.

  • You can go back and change individual answers as needed afterward, but it will pop up a message confirming that this will override any answers already entered and ensuring you want to do this:

6. You can also add Photos, Notes and Corrective Action to any question.

  • Photos can either be taken through your device, or uploaded from your camera roll.

  • Notes can be added to further explain your findings.

  • Corrective action can be assigned so issues can be fixed.

NOTE: Some or all of these three options may be required if a question is marked as X - Unacceptable. You will need to complete each before the audit can be submitted. This is set up by your company when creating the audit template.

7. If assigning a corrective action, fill out the form that is presented, noting any required (red *) fields.

8. When you have completed the audit, hit Submit at the bottom.

  • NOTE: Audits are locked after submission, so please ensure you have answered all questions completely and thoroughly before saving:

9. Once a Safety Audit has been submitted, Supervisor users and above can see the summary, filter the questions, and view all answers/photos/notes/corrective actions .

Using an Equipment QR Code to Begin an Audit

  1. Log into your iReportSource mobile app

  2. Tap QR Scanner:

3. Scan the QR code that is posted on or near the equipment:

4. You will be taken to the Basic Information page for the equipment. Tap the drop down menu at the top of the page and select Safety Audits:

5. You will be presented with a list of all Safety Audit templates that are tied to this piece of equipment. Select the one you need to complete:

6. Confirm the establishment and area are correct, and then tap Submit at the bottom of the screen:

7. Complete the Audit as you normally would, and tap Submit when you are finished answering all questions in all sections. Navigate the audit using the Section menu or the Back/Next buttons.

NOTE: Some questions may require additional information such as Notes or Photos or Corrective Action assignment if marked as X/Unacceptable. These are required to do in order to submit the audit.

8. Once finished, from the Summary page tap Submit.

9. Confirm your submission and select any users you may need to email the completed inspection to. Tap Submit when done. The audit is now complete.
