Using a QR Code to Begin an Audit

  1. Log into your iReportSource mobile app

  2. Click the menu and then click QR Scanner:

3. Scan the QR code that is either available with the equipment, or is posted in your work area:

4. You will be taken to the Audit assigned to that QR code. Answer the questions with the appropriate answer.

  • Select the location you are performing the audit under (Group/Establishment and Area)

  • You can add photos, notes, and assign corrective action.

  • You can use the Jump To Section menu at the bottom of the page to jump to different sections of the audit, if it has multiple, or use the Back/Next buttons to quickly move between them:

5. After all parts have been answered, you will see the summary page. On this screen, click Submit. If you are not ready to submit, click Save for Later and it will be available under either the Safety Audits or the Quality Audits section of your My ToDos.

You have successfully completed your Audit using the QR Scanner!

Starting an Audit on Demand (Mobile)

Both Safety Audits and Quality Audits take you through a series of statements/questions where you will answer a series of questions in order to document items that need attention. You are also able to create corrective action for unsafe items, add photos, and add notes detailing what is found.

This document takes you through the steps of starting/completing these audits on demand through the mobile app without the audit needing to be assigned to you.

NOTE: Safety Audits and Quality Audits function the same way when starting on Demand. The example below will feature Safety Audits.

  1. Log into the iReportSource mobile app

2. Click on the + icon or click your menu and choose Action Items:

3. Choose Make Observation:

4. Choose either Safety Audit or Quality Audit and click Submit to confirm your selection:

5. Choose the appropriate Group/Establishment and Area (optional) to associate with the audit:

6. Select the Audit you'll be completing from the list of templates:

7. Click Confirm when you have filled out all fields, and you will be immediately taken to where you can begin completing the audit as usual:

Completing Assigned Audits on the Web

Both Safety Audits and Quality Audits take you through a series of statements/questions where you will answer a series of questions in order to document items that need attention. You are also able to create corrective action for unsafe items, add photos, and add notes detailing what is found.

This document takes you through the steps of completing an Audit that has already been assigned to you on the web platform.

NOTE: Safety Audits and Quality Audits function the same way when accessing and completing an assigned audit. The example below will feature Safety Audits

  1. Log into iReportSource at https://www.ireportapp.com/login

  2. Click on your location on the first page:

3. You will see your assigned Audits as tasks on your My To-Dos dashboard. Click on the Audit you’d like to complete:

4. There is the possibility that each individual question has a different answer type. These answer types range from the Acceptable/Unacceptable/Not Applicable questions, to dropdown, multi-select, text field, employee list, and a signature box. Be aware of this as you are answering the questions! In the example below, you will see a Dropdown and a Text Field answer type.

5. Some audits may also have at the top of the section a Mark all in section drop-down, which allows you to quickly mark all √, X, N/A questions in that section with one of the √, X, N/A options.

  • NOTE: This will override any √, X, N/A answers already provided in the section. You can go back and make individual changes as necessary:

  • NOTE: Based on how the template for the Audit was set up, your organization may require the addition of photos, notes, and/or corrective actions for any item marked as unacceptable.

6. You can click on the Photo button to add pictures. You will be taken to your local file browser to choose the file to upload. You can add multiple pictures - if you do, you will see arrows on the picture display indicating there are more to view:

7. You can add notes to the question response by clicking on the Note icon. A notes box will appear and you can type them in, and hit Add Note. The note will then be visible on the question:

NOTE: If you entered a note in error, you can delete it before submitting the audit.

8. If any Unsafe item needs corrective action, it can be assigned a Corrective Action during or after the submission of the Safety Audit. If you’re ready to assign the corrective action, just click the button under the appropriate question:

If you assign a Corrective Action, you can, during that process, add additional description or notes, select the Priority, a Due Date/Time, and the Assignee(s) for the corrective action, and then hit Assign Now. When you do this, the assignee(s) receive an email that a task has been assigned to them, and it will show up on their dashboard when they next sign into iReportSource.

10. After corrective action has been assigned, it will show under the appropriate question on the Audit:

11. After you have answered the questions, you also have the option to add notes for the whole audit at the end before submitting:

12. When you have completed the audit, hit Save at the bottom.

  • NOTE: Audits are locked after submission, so please ensure you have answered all questions completely and thoroughly before saving:

13. Once an Audit has been submitted, Supervisors and above can see the completed audit. This shows the summary, photos/notes, answers, and scores for the section(s). You can always come back to submitted Audits by going to Audits > Safety Audits or Quality Audits on the dashboard menu and then viewing either the Severity Review or Closed tabs:

1. Log into the iReportSource mobile app

2. Click on either Safety Audits or Quality Audits, depending on what you have assigned to you, from your My To-Dos list:

3. You will be given the list of all assigned Audits for the type you chose. Click on the one you’d like to complete:

4. The Instructions (if any) will show at the top of the page in the Overview section.

5. You have the option to Jump to a Section at the bottom of the page. If the Audit has multiple sections, you’ll see them listed here for easy navigation:

6. There is the possibility that each individual question has a different answer type. These answer types range from the Acceptable/Unacceptable/Not Applicable questions, to dropdown, multi-select, text field, employee list, and a signature box. Be aware of this as you are answering the questions! In the example below, you will see a Dropdown and a Text Field answer type.

You can go back and change individual answers as needed afterward, but it will pop up a message confirming that this will override any answers already entered and ensuring you want to do this:

8. You can add notes to the question response by clicking on the Note button. A page will appear and you will need to click Add Note. A text field will appear. Enter your notes, but be sure to hit Submit when finished, otherwise your note will not be saved! The note will then be visible on the question:

9. You can click on the Photo button to add pictures. You will be given the option to choose the file to upload from your camera roll (you may be asked for permission to access your photo library on your device), or you can take photos on the spot with your device camera. You can add multiple pictures - if you do, you will be able to scroll through the photos by swiping.

10. If any Unsafe item needs corrective action, it can be assigned during or after the submission of the Safety Audit. If you’re ready to assign the corrective action, just click the Assign Corrective Action button under the appropriate question:

If you assign a Corrective Action, you can, during that process, add additional description or notes, select the Priority, a Due Date/Time, and the Assignee(s) for the corrective action, and then hit Submit. When you do this, the assignee(s) receive an email that a task has been assigned to them, and it will show up on their dashboard when they next sign into iReportSource.

12. After corrective action has been assigned, it will show in the list of questions on the Safety Audit. The assignee will also receive a notification that they have been assigned a task, and it will be on their My To-Dos list each time they log into iReportSource.

13. When you have completed the audit, hit Submit at the bottom.

  • NOTE: Audits are locked after submission, so please ensure you have answered all questions completely and thoroughly before saving:

14. Once a Safety Audit has been submitted, Supervisor users and above can see the summary, filter the questions, and view all answers/photos/notes/corrective actions when on a desktop or laptop computer.

  • Audits are not designed to be viewed on the mobile app once completed.

Using an Equipment QR Code to Begin an Audit

  1. Log into your iReportSource mobile app

  2. Click the menu and then click QR Scanner:

3. Scan the QR code that is posted on or near the equipment:

4. You will be taken to the Basic Information page for the equipment. Click the drop down menu at the top of the page and select Safety Audits:

5. You will be presented with a list of all Safety Audit templates that have been chosen to tie to this piece of equipment. Select the one you need to complete:

6. Confirm the establishment and area (optional) are correct, and then hit Submit at the bottom of the screen:

7. Complete the Audit as you normally would, and hit Submit when you are finished answering all questions in all sections. See this article for more information about how to complete Audits on the mobile app.
