NOTE: Only administrator users have the ability to add/edit custom report questions.
Adding Custom Questions
Log in on the web at https://www.ireportapp.com/login
2. Choose your location to go to your dashboard
3. Navigate down the left side to the Incidents menu and select Custom Report Questions.
4. Click on the report type you wish to add custom questions to:
5. Choose which report component the question(s) should be added to for the chosen report type:
6. You’ll be taken to the question creation page. The following fields will be available to you:
Question: This is a required field
Required: Use this if you want to require an answer to this question
Type of Input: How the question can be answered. Choose one of the following:
Dropdown (choose one from a list of possible answers)
Multiselect (select multiple answers from a drop-down menu)
Radio Button (Only allows two answer choices)
Check Box (allows multiple answers to be checked from a list)
Google Address (if you are requiring an address be entered)
Text Field (one line of open text to be entered)
Text Area (multiple lines of open text to be entered)
Date (enter only a specific date)
Date/Time (enter both a specific date and specific time)
Signature Box (field that can be signed with mouse or finger)
Add Not Applicable Checkbox: This will make a Not Applicable option available for this question.
Add Row/Add Five Rows: Click one of these buttons to add one or five more rows to add more questions.
7. The Dropdown, Multiselect, Radio Button and Check Box input options all require pre-determined answers. Click on View Options to bring up a window to input those answers.
NOTE: You will always have to input at least one answer, regardless of which of the four above options is chosen. To add more than one answer, select Add Option as many times as you need and fill in the answer(s) where it says Type your option here.
NOTE: Once you have entered the available options, be sure to hit Save Changes to save your answers.
8. Once you have your question(s) and answer fields entered, hit the green Create button at the bottom of the page:
9. Moving forward, you will find a section entitled Custom Questions which will include all questions you created for that particular component of the report type chosen in Steps 4/5.
NOTE: These changes will NOT impact previously created reports. Only new reports created after the custom questions are added will see the change.
Editing Existing Custom Questions
1. To edit or delete any of the custom report questions you have made, follow steps 1-5 in the previous section to access the report component that now has the question(s) you wish to edit or delete.
2. Once in that report component, you will see in the top right corner the option to Edit Report Questions. Click there.
3. You will then have the option to change the question itself, its answers, or to add additional questions.
NOTE: You cannot change the input type for the question answer, whether it is required, or the appearance of a Not Applicable checkbox. If you want to change these settings, it is recommended to clone the question and then make the necessary adjustments, and delete the original question. (See next section).
4. To clone a question, click on the clone icon while hovering over the question:
5. You can change any of the question information. If you don’t want to keep the original, be sure to delete it.
Deleting Existing Custom Questions
1. To delete a question, click on the red X next to the clone button while hovering over the question:
2. When finished making edits/clones/deletions, click Save at the bottom of the page.
NOTE: These changes will not be retroactive -- only new reports moving forward will have the updated custom questions.
3. To delete the entire list of questions at once, click on the red Delete Report Questions button in the top right corner of the screen. You will then be asked to confirm your decision. Once confirmed, the entire list of questions will be deleted.
NOTE: Only reports moving forward will see this change.
