NOTE: Only Admins have the ability to create and edit Behavior Observation Templates
Behavior Observations are filled out while observing an employee performing a job/task, identifying any areas for safety improvement and ensuring all procedures are followed. You are also able to create a corrective action for follow up items, add photos/documents, and add notes.
This document takes you through the steps of creating a behavior observation template needed to perform the Behavior Observation.
Templates are used in order to provide your company with the flexibility to ask the questions they want answered, with the ability to change those questions as time goes on.
1. Log into the iReportSource Web App on your desktop or laptop.
2. On the left navigation bar, click Observations then Behavior Observation Templates.
3. The next page you see will be the main Behavior Observation Templates page. This page lists out all of your created templates. It is from here that you will create new templates, duplicate existing ones, and search for existing templates in order to edit or delete.
Creating a New Template
1. From the main Behavior Observation Templates page, click in the top right corner where it says Page Actions and select from the drop down menu Add Behavior Observation Template.
2. You will be taken to the template creation page. All Behavior Observation Templates start with fields for the Title and Instructions.
Each Observation will automatically have fields for Employee Observed, Supervisor Observing, Date Observed and Job task, so you do not need to add these.
3. To add your questions, you will click on Add Row and/or Add Five Rows, depending on how many questions you wish to add.
4. Each question has the ability to be identified as a Required Question (preventing submission until answered), can allow for additional comments, and can be formatted with BOLD text, Italic Text, bullet points, numbered lists, or just regular text.
5. Each question also has the following Input (question/answer) types:
Dropdown (single select answer from a list)
Multiselect (multiple select answers from a list)
Radio Button (Two total answer options, single select)
Checkbox (single select answer from a list)
Text Field (short, typed answers)
Text Area (long form, typed answers)
Date (calendar date of choice)
Date/Time (calendar date of choice, plus time of choice (00:00 format))
Block of Text (Non-answered question. Any text in the Question field is treated as a standalone statement. Great for breaking the template into sections)
Employee List (multiselect list of employees entered into iReport)
Signature (a box for capturing a signature or initials)
6. Once you have added all your questions and are satisfied with the template, hit the green Create button at the bottom of the page. You can now go back to the main Behavior Observation Templates page. Your template will now appear on the list of templates and can now be used when a Behavior Observation is to be completed.
A completed template, ready for creation, should look something like this:
Managing Existing Behavior Observation Templates
1. Log into the iReportSource Web App on your desktop or laptop.
2. On the left navigation bar, click Observations then Behavior Observation Templates.
3. The next page you see will be the main Behavior Observation Templates page. This page lists out all of your created templates.
4. Select which template you wish to manage.
5. Once selected, you will find that the template is locked. To unlock it, click the blue Edit Template button in the top right corner.
6. The template is now unlocked and you can edit the title and instructions, as well as add additional questions.
At anytime during the editing process, you can also outright Delete the entire template. After entering the edit mode, simply click on the Delete Template button in the top right corner. You will be asked to confirm your choice, and then the template will be deleted.
7. You will notice, however, that the Input (question/answer) type, Is Required, and Allow Additional Comments components are still locked.
This is to help prevent accidental changes to these components.
If you need to change any of these, then you will need to clone the question itself by hovering your mouse in the top right corner of the question and then clicking on the Blue box that appears.
8. Once you have made your changes, simply click on the blue Save button at the bottom of the page. Your changes are now saved and will be reflected on Behavior Observations moving forward.
Duplicating Existing Behavior Observation Templates
1. Log into the iReportSource Web App on your desktop or laptop.
2. On the left navigation bar, click Observations then Behavior Observation Templates.
3. The next page you see will be the main Behavior Observation Templates page. This page lists out all of your created templates.
4. In the top right click on Page Actions and select Duplicate Behavior Observation Template.
5. A modal will appear. From the dropdown list, select which template you wish to duplicate. Hit Create once you've made your selection.
6. The template will now appear and all fields will be unlocked. Click Create at the bottom of the page once you are done.
NOTE: The title will show "- Copy" in it. You will want to change the title to reflect your changes.
7. You will now see your duplicated template on the main Behavior Observation Templates page. You are all done!
