Supervisors, Collaborators, and Administrators who have access to an incident report can assign witness statements to anyone via email. This is helpful for when you need a witness statement from a non-employee, an employee who does not have an active iReportSource account, or perhaps an employee who does not work in the establishment the incident is under.
It is important to note that the link you send out to them via email will allow them to fill out a Witness Statement for 72 hours only. If they do not submit it within the 72 hours, they can request an extension which will come to you to approve.
Sending Witness Statements from the Web
From the Investigation tab of the Incident Workflow, click into Witness Statements:
2. Click on Page Actions in the top right corner, then Add External Witness:
3. Enter the Contact Information (for the person collecting the statement) and the Witness Information (for the person filling out the statement form). Click Add Row to add more Witnesses, or click Submit to send the Witness Statement email.
4. At this point, the email goes out to the email you provided and the 72 hour time frame begins for the statement to be submitted. You will see the status of the statement both on the Incident Witness Statements page or on the Witness Statements list from the menu:
