Using a QR Code to Begin a Custom Document
Log into your iReportSource mobile app
Click the menu and then click QR Scanner:
3. Scan the QR code:
4. You will be taken to the blank Custom Document to begin filling it out immediately:
You can take or upload photos or upload files to accompany the document
5. After all required questions have been answered, you will be able to click Submit at the bottom of the page.
6. Once you are taken to the list of completed documents, you will see yours there. Do not close the app until you get to the list of completed Custom Documents:

Creating on the Mobile App
1. Log into the iReportSource app
2. Click on the menu icon:
3. Choose Documents:
4. Click Custom Documents:
5. You are taken to a page that shows all previously-submitted custom documents for your location. Click the + icon at the bottom right corner to create a new document.
6. Choose the document you want to create -- the list of custom document templates will be displayed:
7. Fill out the form, starting with your Group/Establishment (and Area/Jobsite if needed) and click Submit when finished:
NOTES:
Required questions are marked with a red asterisk *
You can add pictures or files from your device (optional)
You can use talk-to-text for any questions that allow for text input

Log into iReportSource at https://www.ireportapp.com/login
Access your Home page by choosing a location
Choose one of the following options to create your Custom Document
Option 1: From the Home Page
Click the Custom Document button at the bottom of your home page and follow the modals that follow:
Lastly, enter answers for all applicable questions in the form (required questions are indicated with a red asterisk). You have the option to upload attachment(s) if necessary. Once you are finished, click Create at the bottom of the page:
Option 2: From the Custom Documents page
1. From the Custom Documents page (Click on Document Repository in the menu to the left, and select Custom Documents), select Page Actions in the top right and then from drop-down menu, choose Create Document.
2. Choose the type of custom document you need to create:
3. Click Create:
4. Enter answers for all applicable questions in the form (required questions are indicated with a red asterisk). You have the option to upload attachment(s) if necessary. Once you are finished, click Create at the bottom of the page:
5. Your newly created document will now be on the list of custom documents when you come back to this area:

Duplicating a Completed Custom Document
Some documents done on a regular basis have only a few changes to record. Rather than needing to fill out the entire blank custom document every time, users have the option to duplicate an existing already-submitted document, change any answers they need to change, and then submit the new copy. To do so, follow the steps in this document:
ON THE WEBSITE
Go to Document Repository > Custom Documents on your left-side menu
2. Find the document you wish to duplicate, and click on it:
3. Click on Page Actions in the top right corner, and choose Duplicate Document:
4. The document will be copied, and all answer fields are now editable, with the previous answers pre-loaded. Make any changes you'd like to make, and then click Create.
NOTE: Signature fields are not populated, so you'll have to add a signature if there are any required on the document before you're able to submit.
ON MOBILE
Go to Documents > Custom Documents from the menu:
2. Find the document you want to duplicate, and click on it:
3. Click on the Duplicate button in the bottom of the screen:
4. The document will be copied, and all answer fields are now editable, with the previous answers pre-loaded. Make any changes you'd like to make, and then click Save.
NOTE: Signature fields are not populated, so you'll have to add a signature if there are any required on the document before you're able to submit.
