Product Release Notes 10/11/26

Updated Oct 12, 2026· Product Release Notes

The following enhancements were released on 10/11/26:

Custom Forms (Web & Mobile)

Build your own forms and checklists. Administrators create templates in the new Custom Forms section of the left menu: drag in sections and fields (text, dropdowns, dates, signatures, photos, tables, people, and more), set fields to show only when an earlier answer matches, and choose who approves a submitted form. A template can be scheduled to repeat, assigned to people with a due date, posted as a QR code that opens a fresh form when scanned, attached to an Incident stage, or linked to a piece of Equipment.

Users fill forms out on the web or in the mobile app, save drafts, and get a PDF, an emailed report, and a CSV export. Three starter templates are included: Job Safety Analysis, Job Hazard Analysis, and a Sign-In Sheet.

Custom Forms > Templates > New Template, or start from a sample.

Visitor Incidents (Web & Mobile)

Let visitors and contractors report an incident from a posted QR code, with no account. Administrators generate a code for each Group and Area under Incidents > Visitor Incidents, choose which incident types it offers, and print the poster. Anyone who scans it fills out a short report in their browser, with up to five photos.

Submissions wait in the reviewer's My To-Dos on web and mobile. Approving one creates the Incident; rejecting it records the reason. Custom incident questions have a new Visible To setting (Employee, Visitor, or Both) so you control what visitors are asked.

Setup: ask your Customer Success Champion to turn on Visitor Incidents, then set the Visitor Incident Administrator under Settings > Company Details.

Preventive Maintenance for Equipment (Web & Mobile)

Track service intervals for Equipment by hours of use. On any piece of Equipment, turn on Preventive Maintenance and enter the current hours, the service interval, the provider (an outside company or your own staff), and the supervisors who approve work. Hours are recorded whenever someone performs an audit on that equipment.

When the interval is reached, a work order goes out automatically: outside providers get a link that needs no login, and your own technicians get a To-Do. The write-up (work performed, parts, meter reading, photos) comes back for a supervisor to approve or send back, from email, My To-Dos, or the equipment's Service Requests list. An optional walk-up QR poster lets a technician check in from a phone camera.

Equipment > open a unit > Preventive Maintenance. Provider and technician rosters live under Equipment > Manage Providers.

Custom Diagrams (Web & Mobile)

Choose which diagrams appear on incident reports. Under Settings > Company Details > Diagrams, administrators can rename or hide the standard Body, Vehicle, and Road diagrams, add their own types with uploaded images, and choose which incident types show each one. The diagram picker on web and mobile follows these settings, and saved diagrams are grouped by type on the PDF. If you change nothing, nothing changes.

Safety Audits: new Perform and Summary pages (Web)

Performing an audit on the web has a new layout: one card per section with a running count of answered questions, Acceptable / Unacceptable / N/A buttons on each question, and note and photo counts at a glance. The summary page shows the overall score, section scores, and Corrective Action counts. This applies to Safety, Quality, and Lockout/Tagout audits; scoring and workflow are unchanged.

Automatic Report Delivery (Web)

Send audit and Custom Form reports automatically. Under Settings > Company Divisions, the new Audits & Custom Forms Report Delivery card lets administrators set rules: for a type of audit or form, in chosen Groups (and optionally Areas and templates), email the finished report PDF to selected employees or outside addresses. Someone on a rule who is also picked by hand receives one copy, not two.

Template Notifications (Web)

Templates for Audits, Behavior Observations, Custom Documents, Custom Forms, and Trainings now have a Notifications section. Pick who is notified, by user level or by name, and for which events. When set, these recipients replace each person's own notification preferences for items made from that template.

Location Selector (Web)

The location button in the top header is now a pill that shows the active location and its parent. Clicking it opens a searchable Company > Group > Area tree, and picking a location refreshes the list you are on. On create, edit, and detail pages the selector is locked so you cannot switch away by mistake.

Incidents: Root Cause custom questions (Web & Mobile)

Custom questions can now be added to the Root Cause section of an incident, using the same question builder as the other sections. Answers appear in the PDF, exports, and the public API. Incidents created before the questions were added are not affected.

Incidents: External Witnesses (Web & Mobile)

While creating an incident, Add External Witness (under Additional Witnesses) records a witness who is not an employee: their name, email, and a contact. When the report is submitted, each one receives the witness statement request with the usual 72-hour window.

Audits: Equipment selection by the assignee (Web & Mobile)

When performing an assigned Safety Audit or Lockout/Tagout whose template is linked to equipment, the assignee now chooses the piece of Equipment the audit is for: below Involved Employees on web, in the Overview card on mobile.

Employee Pickers (Web & Mobile)

Employee pickers show each person's job title and a status pill (Full Time, Terminated, Deactivated, and so on). Fields that record history, such as the injured worker or meeting attendees, can include former employees behind a Current Employees Only checkbox. Fields that assign work never offer former or deactivated people. A name already saved on a record stays visible when you reopen it.

Equipment: Repeating Tasks and Information card (Web)

The Tasks card on an Equipment record has Active and Repeating tabs, and you can create a repeating Task directly from the equipment; each occurrence links back to it. The Information card is reorganized into Identification, Location, and Lifecycle & Cost, with the status shown as a pill next to the title (change it with Edit).

Home Page and Default Incident Owner (Web)

The Home page is now a directory: your company logo above a single card listing Groups, with the number of Areas in each. Administrators can set a Default Incident Owner per Group from the cog on each row, and the company-wide default has moved to Settings > Company Details.

Report PDFs (Web & Mobile)

Report PDFs have a new design across the platform: Incidents, Behavior Observations, Safety Audits, Safety Walks, Trainings and certificates, Custom Documents, Safety Meetings, Custom Forms, Claims, and the Analytics dashboard. Every report now shows Report created with the date and time in the top-right margin, so a printed copy can be told apart from a later version. Emailed audit PDFs include the summary block.

Public API

For customers on the API plan, four more areas are available, all read-only:

  • Equipment: GET /v1/equipment, /v1/equipment/:id, and /v1/equipment/:id/maintenance (preventive maintenance details and work orders)

  • Custom Forms: GET /v1/custom-forms, /v1/custom-forms/:id, and /v1/lookups/templates/custom-forms

  • Claims: GET /v1/claims and /v1/claims/:id (access follows the user's in-app claims access; no Social Security numbers)

  • Training courses: GET /v1/courses, /v1/courses/:id, /v1/courses/export, plus courseId on /v1/trainings

Keys can now be sent in an X-API-Key header as well as the Authorization header, and new keys can be scoped to equipment, Custom Forms, and claims separately.

See the API documentation for request and response details.

And more

  • Page headers across the web app put actions like Edit, Delete, and Generate inline instead of in a menu, with a back link.

  • Buttons, dialogs, dropdowns, and upload windows have a consistent new look, and the web app uses the Inter font on every operating system.

  • Settings > Areas and the Groups list use a new table layout.

  • Audit perform: the section's Mark all control is a dropdown (Acceptable / Unacceptable / N/A).

  • Emailed audit PDFs include the Summary block, and the cover email names who created and who was assigned.

  • Incident "Email Initial Report" uses the new PDF.

  • Mobile: the location pill shows the parent location above the active one.

  • Equipment page: a Custom Forms tab, plus Preventive Maintenance and Service Requests cards.

  • Custom Forms favorites keep working after a template is republished.

  • Public pages reached from QR codes use invisible reCAPTCHA, so there is no puzzle to solve.
